Annual Report Forms:

Every member of The Saginaw Chippewa Indian Tribe of Michigan must fill out an Annual Report Form which is due by May 4th each year. The forms must be signed in the presence of a Notary Public and then filed with the Tribal Clerk’s Office. Members who fail to file Annual Report Form will not be entitled to any benefits, including monetary, that may accrue to members of the Tribe.

    Considerations:

    The Tribal Children member's parent/guardian has the sole responsibility of obtaining, completing, and submitting any/all applications for the Tribal Children's Welfare Program. All applications must be received, notarized and in original three part form by mail or person by 5 p.m. on the due date . .. no faxes ... no exceptions. Postmark date does not and will not apply to the due date. All forms must be completed in full or will not be accepted ...

    Parents/guardians applying for payment of children in which the 51% custody requirement for the payment period has not been met are subject to legal prosecution.

    Newly enrolled children must have an enrollment date before the beginning of the quarter to be eligible for that quarter’s payment.

Legal Guardianship Status

When the legal guardianship for Tribal Member Children changes, proper documentation is required. Child welfare payments will be sent to the person who is the legal custodial parent or guardian of the listed children and who will attest that they provide at least 51% of the child’s support and that the child resided with them 51% of the time during the last quarter. These forms must be signed in the presence of a Notary Public and then filed with the Tribal Clerk’s Office.

Address Change Form

When Tribal Members wish to change their mailing and/or residential address, they are required to file a change of address form which must be signed in the presence of a Notary Public and filed with the Tribal Clerk’s Office. The form also changes their address for voting purposes when they are a registered voter. The form must be filed with the Tribal Clerk’s Office 10 days prior to the first of the month to ensure that Pre Capita checks are sent to the new address.

Change of Address Form for Tribal Children Members

To change an address for a Tribal Member who is a child (under the age of 18 years old), this form must be filled out by a custodial parent or legal guardian. The parent or guardian, by signing the form, will attest that the child legally resides at the address listed.

Application for Absentee Ballot

Any registered voter may apply for an Absentee Ballot if they meet one of the requirements for an Absentee Ballot within Ordinance #4. To get an Absentee Ballot, the completed form must be signed by the registered voter requesting the Absentee Ballot. The application can either be mailed or brought in to the Tribal Clerk’s office before the day of the election. Absentee Ballots will be mailed to the registered voter in a manner that will ensure they are received in time for the stated election. Absentee Ballots will not be given to any person other than the registered voter who signs the request. Absentee ballots must arrive by the close of voting for the General Election which is 8 p.m.

Voter Registration Form:

The Voter Registration Form is specifically designed to add Tribal Members to the official list of eligible voting members. The form does not change the member’s address and their residence address must match the address on their voter’s registration. If the Tribal Member’s residential address has changed they must use a Change of Address form. To be eligible to vote in the General Election, Tribal Members must be registered to vote by 30 calendar days before the election.
