Adult Mno Shkiziwin Direct Deposit Form
Mno-Shkiziwin Direct Deposit form is to be filled out if you wish to have your bi-weekly supplemental assistance funds sent directly to the bank instead of receiving a paper check in the mail. This form takes two pay periods to start from the time it is received in our office. This form may be email, mailed, faxed or dropped off in person.

Direct Deposit form is to be filled out if you wish to have your bi-weekly funds sent directly to the bank instead of receiving a paper check in the mail. This form takes two pay periods to start from the time it is received in our office.  This form may be mailed, emailed, faxed or dropped off in person.

Stop Direct Deposit Form:
Stop Direct Deposit form is used to stop a direct deposit from going to a specific bank account.
This form may be mailed, emailed, faxed or dropped off in person.

Michigan Income Tax Form:
This form is used to have a portion of each of your bi-weekly checks withheld and payments made to the State of Michigan for income tax purposes.
This form may be mailed, emailed, faxed or dropped off in person.

Federal Income Tax Form:
This form is used to have a portion of each bi-weekly check withheld and payments made for Federal income tax purposes.
This form may be mailed, emailed, faxed or dropped off in person.

Authorization for Release of Per Capita Information form:
This form must be filled out prior to the release of any Per Capita information such as Verification Letters, 1099s, loan histories, check stubs or any account history. The Release form is processed in the order it is received and my take three to five business days to complete your request. This form may be emailed, mailed, faxed or dropped off in person.

Voluntary withholding form:
This form is to have funds voluntarily withheld on a bi-weekly basis from your Per Capita check for deductions to Bankruptcy Courts, Federal Courts, IRS, Isabella County Courts or Saginaw Chippewa Tribal Court.This form may be mailed, emailed, faxed or dropped off in person.

Pay off Loan Deduction Form:
This form is to increase your bi-weekly loan payment.
This form may be mailed, emailed, faxed or dropped off in person.

Loan Application and Guidelines 022824:
Loans are available to tribal members of the Saginaw Chippewa Indian Tribe of Michigan, eighteen years of age and over. Mailed and emailed applications must be notarized. No Faxed applications will be accepted.
